If you ask me what the most challenging cosmetic condition to treat is, I would have to put cellulite up there on top of the list. Statistically, over 90% of women in the U.S. suffer from some degree of cellulite in their thighs and buttocks. Like so many things that are unfair, it seems to be a problem that specifically affects women. The reason is that for some reason, fibrous bands vertically connect the skin to the underlying fascia on the female body whereas, the bands form horizontally in men, sparing the males of our species from this pesky condition. There have been plenty of treatments from creams to lasers to injections, that promised to eliminate cellulite. Unfortunately, pretty much all of the treatments had failed to deliver long term improvement for cellulite.
Endermology, a treatment that uses suction and massage to stretch the bands gave some improvement, but it was temporary. To maintain results, women would have to commit to weekly treatments which was really impractical. Treatments like Velashape® and SmoothShapes® combined the massage and suction with radio frequency and laser energy to help the results last longer, but these treatments were still temporary and often too costly to maintain. Plus, who has the time to do this? Cellulase® was a laser that fired sideways so that theoretically, we could cut the bands with a laser. Cellfina® was a device that inserted a small vibrating blade under the skin to cut the bands. Quo® was a simple injection that utilized an enzyme to try to dissolve the bands. The big drawback that all these treatments shared was that they were not precise. You can insert a probe, a blade or a needle in the area of the dimple, but the actual cutting was done blindly. It's like locating a school of fish and trying to catch one with a little net. Like the fish, the cellulite bands are awfully evasive.
Recently, an innovative new device was introduced. This tool is different from all the others in that it uses an LED light to locate the bands under the skin and then deploys a little hook to allow the surgeon to capture the band. Once the band is caught, there is no mistake. You can see the dimple become exaggerated as you tug on it. A simple press of a button extends a tiny blade to precisely cut the band and "pop" you can actually see and feel the band release and the dimple disappears for good! Such a simple but elegant invention that actually works.
I have used all the previous treatments that I mentioned and stopped offering them because I just didn't feel that the temporary results were worth the money or the time commitment for my patients. The Aveli® is the first cellulite treatment that "wowed" me. The only other device that made me say "wow" was the SylfirmX® which we introduced a few years ago for treating skin laxity. Naturally my inclination was to see what happens when we combine the two treatments so we did just that. The verdict? "Wow, and wow!" The Aveli® tool allows me to offer a long term solution for the cellulite dimples and the SylfirmX® tightens the loose skin for a truly winning combination for my patients.
